Project Description
The Northern Cheyenne tribe is conducting a Health and Housing Needs Assessment to collect up-to-date, accurate information about the characteristics, needs, and priorities of the households and communities located on the Northern Cheyenne Reservation. Tribal leadership will use this data to inform strategic planning and prioritize community needs and concerns in the development and delivery of programs and services.
Since the information gathered will be kept confidential and will not identify any specific individuals or households, this information will support community-level decision-making as opposed to the delivery of specific services or benefits at the household level.
Raffle Rules
Households that have COMPLETED a survey before 12pm (noon) on the day of the raffle are eligible for the prizes offered.
Raffle drawings will be livestreamed on Facebook for community members to watch.
If you are a winner, bring your door hanger with your survey code to the Northern Cheyenne Tribal Housing Authority and claim your prize.

This survey is funded by a grant provided through the Montana Healthcare Foundation, a statewide foundation with a branch that supports Native-led solutions to improving the quality and availability of health care, and the Wells Fargo Invest Native Grant, funds which are committed to advancing opportunities in Native American communities working with Native-led organizations.
The survey is being carried out by the Northern Cheyenne Tribal Housing Authority with support from various other Northern Cheyenne departments and programs.

The Needs Assessment is surveying a random sample of all residential structures/housing units on the Northern Cheyenne Reservation.

Each randomly chosen household will be visited by a field staff member in an orange vest during late August to drop off a door hanger. THE SURVEY CODE IS LOCATED ON THE BACK OF THE DOOR HANGER.

To show our appreciation for your participation in the needs assessment survey, each household that COMPLETES a survey will receive a $20 gift card. Additionally, you will be entered into a raffle to win larger prizes.

The survey asks general questions about your income, employment, health, and demographics, as well as specific questions regarding your interest in new housing, infrastructure, economic development, and supportive services.
We encourage you to answer every question; if you don't know the precise answer to any question, please give your best estimate.

The responses you provide will remain anonymous and your name will not be collected as a part of this survey. All of your responses will be held and kept confidential. Your eligibility for services or assistance cannot and will not be impacted by your answers or by your refusal to participate.
The data from this project will only be reported as a collective, combined total, and no one will have access to your individual information.

The purpose of the survey is to collect accurate information about the households and residents of the Northern Cheyenne Reservation so that the Tribe can develop programs and services to meet your needs.
The tribe will use the data collected in this survey to apply for state and federal programs supporting the development of housing and other services. The tribe will also use the information to make decisions about the Tribe’s priorities for the future.
The Housing Authority will share the results of the survey with the community at the conclusion of the survey.
