A unique survey code is often used to anonymize survey responses and prevent duplicate responses across multiple survey platforms. While survey codes are useful, survey participants often forget their unique code. The above custom dashboard, created using R Shiny, helps survey participants retrieve their unique survey code using their tribal ID and date of birth.
